•  Early intervention to determine diagnosis and medical status.
• Injuries that require extensive diagnostic testing and imaging
• To assist with coordinating and following up with extended therapy visits
• If an injured worker has a severe laceration.

• Catastrophic and/or multiple injuries
• Surgery is requested.
• Evidenced based guidelines for treatment have been exceeded.
• Multiple treating providers
• Comorbid Conditions – Injured workers with obesity, hypertension, diabetes, or other comorbid conditions are at higher risk for complications. Case managers will address these comorbidities with providers and patients to help avoid unnecessary complications.

Case managers are the centralized figures that communicate with all parties to include providers, employer, injured worker, attorneys, claims adjuster, and ancillary services. They ensure appointments are scheduled appropriately and avoid delay in care which in turn leads to timely return to work. They provide education to patients related to their treatment plans to ensure compliance.

Case managers will work with the employers to determine if modified or transitional work is available. They will actively communicate this availability to the providers and advocate for return to work at these levels when appropriate. If the injured worker is not able to return to work at the job of injury a vocational counselor may be appropriate to evaluate wage earning capacity or job placement depending on jurisdiction.
